
Clinical Pharmacist
We are seeking an experienced and highly motivated Clinical Pharmacist to join the Northwestern University Health Care team. This is an excellent opportunity to provide pharmaceutical care services to patients within a world-renowned academic health system. The ideal candidate is a compassionate healthcare professional with strong knowledge of pharmaceutical services, patient-focused care, and the ability to work collaboratively with a multidisciplinary team. The successful candidate must possess a Doctor of Pharmacy degree, along with a valid state license. Additional qualifications include a minimum of three years of clinical pharmacy experience, excellent communication and interpersonal skills, and the ability to work independently. The successful candidate will also have experience in providing pharmaceutical care services within a hospital or health system setting.
Responsibilities: • Develop and implement comprehensive pharmaceutical care plans for patients • Collaborate with physicians, nurses, and other healthcare professionals to ensure optimal pharmaceutical care outcomes • Monitor patients for drug interactions, side effects, and therapeutic outcomes • Provide clinical recommendations for drug therapy selection and dosing adjustments • Evaluate patient medication histories for cost-effective and safe drug therapies • Educate patients and families on safe and effective use of medications • Monitor medication compliance and adherence • Prepare and dispense medications in accordance with relevant regulations • Maintain accurate and up-to-date patient records • Participate in drug utilization review and other quality improvement initiatives.

Phd. In Pharmaceutical Sciences Or Related Field
