- Dispensing and reviewing medications accurately and efficiently to ensure patient safety and adherence to medical prescriptions.
- Collaborating with healthcare providers to ensure appropriate medication therapy for patients.
- Providing drug information and counseling to patients and their families on proper medication use, potential side effects, and interactions.
- Maintaining accurate and up-to-date records of all dispensed medications and patient profiles.
- Monitoring and evaluating patient responses to medication therapy and making necessary adjustments.
- Adhering to all regulatory requirements and guidelines related to medication dispensing and storage.
- Participating in medication reconciliation processes to ensure continuity of care for patients.
- Maintaining inventory levels of medications and supplies, ordering and restocking as needed.
- Identifying and reporting any potential medication errors or adverse drug reactions.
- Staying updated on current pharmaceutical developments, new drugs, and treatments.
- Providing training and guidance to pharmacy technicians and other support staff.
- Collaborating with other healthcare professionals to develop and implement protocols and policies related to medication safety and effectiveness.
- Maintaining a professional and compassionate demeanor while interacting with patients, colleagues, and healthcare providers.
- Contributing to quality improvement initiatives and participating in performance improvement activities.
- Upholding the mission, vision, and values of Northwestern Medical Center in all interactions with patients and colleagues.
Doctor Of Pharmacy Degree (Pharmd) From An Accredited School Of Pharmacy.
Valid License To Practice Pharmacy In The State Of Vermont.
Minimum Of 2 Years Of Experience Working As A Pharmacist In A Hospital Or Healthcare Setting.
Strong Knowledge Of Pharmaceuticals, Drug Interactions, And Dosage Calculations.
Excellent Communication And Interpersonal Skills To Effectively Interact With Patients, Physicians, And Other Healthcare Professionals.
