  1. Provide compassionate and high-quality care to patients during their transition from hospital to home.
  2. Ensure continuity of care by coordinating with healthcare providers and other members of the patient's care team.
  3. Advocate for the best interests of patients and promote optimal health outcomes.
  4. Utilize strong clinical knowledge to assess patient needs and develop appropriate care plans.
  5. Communicate effectively with patients, families, and healthcare providers to ensure understanding and cooperation during the transition of care process.
  6. Collaborate with interdisciplinary teams to identify and address any barriers to successful transitions of care.
  7. Monitor and track patient progress, identifying any potential issues or concerns and taking appropriate action.
  8. Educate patients and their families about their care plans, medications, and any necessary follow-up appointments.
  9. Maintain accurate and up-to-date documentation of patient care and progress.
  10. Stay current with industry standards and best practices in transition of care, and share knowledge and expertise with colleagues.
  11. Participate in quality improvement initiatives and provide feedback for process improvement.
  12. Adhere to all regulatory requirements and company policies and procedures.
  13. Maintain a professional and collaborative working relationship with colleagues, patients, and other healthcare professionals.
  14. Attend training and educational opportunities to continuously enhance skills and knowledge.
  15. Adhere to ethical and legal standards of nursing practice.

Job Qualifications
  • Active Rn License In The State Of Employment: The Candidate Must Hold An Active And Unrestricted Rn License In The State Where The Job Is Located.

  • Minimum Of 3 Years Of Clinical Experience: The Ideal Candidate Should Possess At Least 3 Years Of Experience Working As An Rn, Preferably In A Clinical Setting Such As A Hospital, Clinic, Or Long-Term Care Facility.

  • Experience In Care Coordination Or Transition Of Care: The Candidate Should Have Experience In Coordinating Care And Managing Transitions For Patients, Preferably In A Healthcare Setting.

  • Knowledge Of Healthcare Regulations And Guidelines: The Candidate Should Have A Strong Understanding Of Healthcare Regulations And Guidelines, Such As Hipaa And Medicare, To Ensure Compliance In Their Role As A Transition Of Care Coach.

  • Excellent Communication And Interpersonal Skills: The Candidate Should Possess Excellent Communication And Interpersonal Skills To Effectively Interact With Patients, Caregivers, And Healthcare Providers To Ensure A Smooth Transition Of Care.
