  1. Conduct comprehensive assessments of members’ behavioral health needs and develop individualized care plans to address those needs.
  2. Collaborate with healthcare professionals, community organizations, and other resources to coordinate and provide appropriate care and services for members.
  3. Monitor and evaluate members’ progress towards achieving their treatment goals and adjust care plans as needed.
  4. Provide ongoing support and counseling to members and their families, offering guidance and resources to improve their overall well-being.
  5. Document all interactions and interventions in a timely and accurate manner, ensuring compliance with all state and federal regulations.
  6. Advocate for members’ rights and ensure they are receiving culturally competent and appropriate care.
  7. Conduct crisis assessments and develop crisis intervention plans to address urgent situations.
  8. Stay up-to-date on current behavioral health trends, best practices, and resources to inform and improve the quality of care provided to members.
  9. Collaborate with interdisciplinary teams to identify and address any barriers to care and advocate for necessary changes to improve the overall health and well-being of members.
  10. Participate in training and development opportunities to enhance knowledge and skills in the field of behavioral health.
  11. Adhere to all Molina Healthcare policies and procedures, as well as all ethical and professional standards.
  12. Maintain strict confidentiality and adhere to all HIPAA regulations in handling sensitive member information.
  13. Exhibit a high level of empathy, compassion, and cultural sensitivity when working with members and their families.
  14. Demonstrate excellent communication and interpersonal skills to effectively engage and build rapport with members, colleagues, and external stakeholders.
  15. Perform other related duties and responsibilities as assigned by superiors.

Job Qualifications
  • Bachelor's Or Master's Degree In Social Work Or A Related Field

  • Minimum Of 2-3 Years Experience In Behavioral Health Case Management Or Related Field

  • Knowledge Of Mental Health And Substance Abuse Disorders, Treatment Options, And Community Resources

  • Strong Communication And Interpersonal Skills To Effectively Collaborate With Patients, Families, And Healthcare Providers

  • Ability To Assess And Develop Individualized Treatment Plans, Monitor Progress, And Make Referrals As Needed To Ensure Comprehensive Care For Patients.
