Job Description

Are you passionate about social and health equity? Are you motivated to use your exceptional communication and interpersonal skills to help others? Molina Healthcare is looking for an experienced Social & Health Equity Navigator to join our team.As our Social & Health Equity Navigator, you will be responsible for engaging members and providers in the healthcare system to ensure access to care for those who are most in need. You will provide consultation and education on issues related to social determinants of health and create and implement strategies to improve the health and well-being of our members.The ideal candidate for this role has a Bachelor’s degree in social work or a related field, and at least two years of experience in a healthcare or social services setting. You must be familiar with healthcare systems, community resources, and social services programs, and have excellent verbal and written communication skills. You should be familiar with trauma-informed care, and have the ability to work independently and manage multiple tasks.If you are passionate about making a difference in the lives of those in need, we encourage you to apply for this position.
