  1. Conduct comprehensive assessments of patients with behavioral health needs to determine appropriate care and treatment plans.
  2. Collaborate with interdisciplinary teams to develop and implement individualized care plans for patients.
  3. Monitor and evaluate patient progress, making necessary adjustments to treatment plans as needed.
  4. Advocate for patients' rights and well-being, ensuring they receive high-quality and culturally competent care.
  5. Utilize evidence-based practices and clinical guidelines to provide effective and efficient care.
  6. Coordinate and communicate with external providers and community resources to ensure continuity of care for patients.
  7. Educate patients and their families on behavioral health conditions, treatment options, and self-care techniques.
  8. Develop and maintain relationships with community partners to enhance service delivery and support patient outcomes.
  9. Document all patient interactions, interventions, and progress in a timely and accurate manner.
  10. Adhere to all ethical, legal, and regulatory standards in the provision of care and services.
  11. Participate in quality improvement initiatives to enhance the overall delivery of behavioral health services.
  12. Stay updated on industry trends and best practices to continuously improve skills and knowledge.
  13. Provide support and guidance to less experienced team members, serving as a mentor and role model.
  14. Foster a positive and collaborative work environment that promotes teamwork, respect, and professionalism.
  15. Contribute to a culture of diversity, inclusion, and equity in all aspects of care delivery.

Job Qualifications
  • Bachelor's Degree In Nursing (Bsn) Or Related Field From An Accredited Institution.

  • Active And Unrestricted Registered Nurse (Rn) License In The State Of Employment.

  • Minimum Of 3-5 Years Of Experience In Behavioral Health Nursing Or Case Management.

  • Certification In Case Management (Ccm) Or Related Specialty (E.g. Psychiatric-Mental Health Nursing) Preferred.

  • Strong Understanding Of Behavioral Health Conditions, Treatment Modalities, And Community Resources.
