  1. Provide exceptional care coordination and transitions of care services to Molina Healthcare members.
  2. Utilize strong background in care coordination to develop and implement individualized care plans for members.
  3. Conduct thorough assessments of members' health needs and develop plans to address any gaps in care.
  4. Collaborate with interdisciplinary teams to ensure smooth transitions of care for members.
  5. Serve as a resource and advocate for members, providing education and support to improve their health outcomes.
  6. Use evidence-based practices to promote quality and cost-effective care for members.
  7. Maintain accurate and timely documentation of all interactions and interventions with members.
  8. Stay up-to-date on current healthcare policies, procedures, and regulations to ensure compliance.
  9. Participate in quality improvement initiatives and provide feedback to improve the effectiveness of care coordination.
  10. Serve as a role model for ethical and professional behavior within the organization.
  11. Act as a liaison between members, healthcare providers, and community resources to ensure continuity of care.
  12. Demonstrate empathy and compassion towards members and their families, understanding the challenges they may face.
  13. Utilize strong communication skills to effectively collaborate with members, families, and healthcare professionals.
  14. Serve as an advocate for underserved communities and contribute to Molina Healthcare's mission of providing quality, affordable healthcare.
  15. Adhere to all legal and ethical standards related to patient confidentiality and privacy.

Job Qualifications
  • Active California Registered Nurse (Rn) License: This Is The Most Crucial Qualification For A Rn - Ca Licensure - Transition Of Care Coach At Molina Healthcare. The Candidate Must Hold A Current And Valid Rn License In The State Of California.

  • Minimum Of 3 Years Of Clinical Nursing Experience: Molina Healthcare Requires Candidates To Have At Least 3 Years Of Clinical Nursing Experience, Preferably In A Hospital Or Acute Care Setting. This Experience Will Ensure That The Candidate Has A Strong Foundation In Patient Care And Clinical Knowledge.

  • Experience In Care Coordination And Transition Of Care: The Ideal Candidate Should Have Experience In Coordinating Care For Patients And Managing Their Transition From One Healthcare Setting To Another. This Includes Experience In Discharge Planning, Follow-Up Care, And Communication With Other Healthcare Providers.

  • Knowledge Of Medicare And Medicaid Guidelines: As A Managed Care Organization, Molina Healthcare Works Closely With Medicare And Medicaid Beneficiaries. The Candidate Should Have A Strong Understanding Of The Guidelines And Regulations Related To These Government-Funded Healthcare Programs.

  • Strong Communication And Interpersonal Skills: The Role Of A Transition Of Care Coach Involves Working Closely With Patients, Their Families, And Other Healthcare Providers. Excellent Communication And Interpersonal Skills Are Essential For Building Relationships And Ensuring Effective Care Coordination.
