  1. Coordinate and manage remote care for assigned members, ensuring timely and appropriate services are provided.
  2. Conduct telephonic assessments and develop individualized care plans for each member based on their unique needs and goals.
  3. Collaborate with healthcare providers, community resources, and other members of the Facilities team to ensure comprehensive and holistic care for members.
  4. Monitor and track member progress, providing support and guidance as needed to ensure successful outcomes.
  5. Advocate for members and assist with navigating the healthcare system, including scheduling appointments and coordinating transportation.
  6. Maintain accurate and up-to-date documentation of member interactions and care plans.
  7. Stay informed about relevant healthcare policies and regulations, and ensure compliance with all necessary guidelines.
  8. Participate in meetings and trainings to enhance knowledge and skills related to remote case management.
  9. Communicate effectively with team members and stakeholders, providing updates and collaborating on care plans.
  10. Demonstrate a commitment to providing compassionate care and promoting Molina Healthcare's mission and values.

Job Qualifications
  • Bachelor's Degree In Social Work, Healthcare Management, Or Related Field.

  • Minimum Of 3 Years Experience In Case Management Or Healthcare Coordination.

  • Proficiency In Remote Communication Tools And Technology, Such As Teleconferencing And Electronic Medical Records.

  • Knowledge Of Healthcare Regulations And Guidelines, Including Hipaa And Medicare/Medicaid.

  • Excellent Communication, Problem-Solving, And Organizational Skills, With The Ability To Work Independently And Manage A Caseload Effectively.
