  1. Provide quality care and support to members enrolled in the IRIS program.
  2. Demonstrate compassion and empathy towards members and their families.
  3. Collaborate with interdisciplinary team members to develop and implement personalized care plans.
  4. Conduct comprehensive assessments to identify members' physical, emotional, and social needs.
  5. Educate members on health management and self-care techniques.
  6. Utilize evidence-based practices to promote optimal health outcomes.
  7. Monitor and document members' progress and response to care.
  8. Ensure timely and accurate completion of all required documentation.
  9. Communicate effectively with members, families, and healthcare providers.
  10. Maintain compliance with all regulatory and organizational policies and procedures.
  11. Participate in quality improvement initiatives to enhance the delivery of care.
  12. Act as an advocate for members, ensuring their needs and preferences are respected.
  13. Stay up-to-date with current healthcare trends and best practices.

Job Qualifications
  • Current Rn License In The State Of Employment.

  • Minimum Of 2 Years Of Clinical Experience In A Healthcare Setting.

  • Knowledge Of Iris (Include, Respect, I Self-Direct) Program And Experience Working With Individuals With Disabilities.

  • Excellent Communication And Interpersonal Skills To Effectively Engage With Program Participants And Their Families.

  • Strong Organizational And Time-Management Skills, With The Ability To Effectively Manage A Caseload And Complete Documentation In A Timely Manner.
