In this role, you will be responsible for providing support to individuals with disabilities and their families, ensuring that they have access to the resources and services they need to live independently and thrive in their communities.   1. Provide support and guidance to individuals with disabilities and their families.
  2. Ensure that individuals have access to necessary resources and services to live independently and thrive in their communities.
  3. Act as a liaison between individuals, families, and service providers.
  4. Conduct assessments to determine eligibility for the IRIS program.
  5. Develop individualized service plans based on the needs and goals of each individual.
  6. Monitor progress and make necessary adjustments to the service plans.
  7. Educate individuals and families about the IRIS program and their rights and responsibilities.
  8. Collaborate with community organizations and resources to expand service options for individuals.
  9. Maintain accurate and detailed records of all services provided.
  10. Stay updated on changes and developments in the IRIS program and related laws and regulations.
  11. Attend trainings and conferences to further professional development and knowledge of the IRIS program.
  12. Maintain a high level of compassion and empathy for individuals and families who may be facing challenging circumstances.
  13. Adhere to all ethical and legal standards in the provision of services.
  14. Work collaboratively with a multidisciplinary team to ensure the best outcomes for individuals.
  15. Continuously seek feedback and evaluate the effectiveness of services provided.

Job Qualifications
  • Bachelor's Degree In Social Work, Human Services, Or A Related Field.

  • Minimum Of 3 Years Of Experience Working With Individuals With Disabilities Or Special Needs.

  • Knowledge Of Wisconsin's Long-Term Care Programs, Including Iris (Include, Respect, I Self-Direct) And Tmg (The Management Group).

  • Excellent Communication And Interpersonal Skills To Effectively Work With Individuals With Disabilities, Their Families, And Community Partners.

  • Ability To Work Independently And Manage A Caseload, While Also Being A Team Player In A Fast-Paced, Collaborative Environment.
