
IRIS Consultant - TMG

- Provide support and guidance to individuals with disabilities and their families.
- Ensure that individuals have access to necessary resources and services to live independently and thrive in their communities.
- Act as a liaison between individuals, families, and service providers.
- Conduct assessments to determine eligibility for the IRIS program.
- Develop individualized service plans based on the needs and goals of each individual.
- Monitor progress and make necessary adjustments to the service plans.
- Educate individuals and families about the IRIS program and their rights and responsibilities.
- Collaborate with community organizations and resources to expand service options for individuals.
- Maintain accurate and detailed records of all services provided.
- Stay updated on changes and developments in the IRIS program and related laws and regulations.
- Attend trainings and conferences to further professional development and knowledge of the IRIS program.
- Maintain a high level of compassion and empathy for individuals and families who may be facing challenging circumstances.
- Adhere to all ethical and legal standards in the provision of services.
- Work collaboratively with a multidisciplinary team to ensure the best outcomes for individuals.
- Continuously seek feedback and evaluate the effectiveness of services provided.
Bachelor's Degree In Social Work, Human Services, Or A Related Field.
Minimum Of 3 Years Of Experience Working With Individuals With Disabilities Or Special Needs.
Knowledge Of Wisconsin's Long-Term Care Programs, Including Iris (Include, Respect, I Self-Direct) And Tmg (The Management Group).
Excellent Communication And Interpersonal Skills To Effectively Work With Individuals With Disabilities, Their Families, And Community Partners.
Ability To Work Independently And Manage A Caseload, While Also Being A Team Player In A Fast-Paced, Collaborative Environment.
