  1. Conduct assessments: Perform comprehensive assessments of individual's long-term care needs, including physical, mental, and emotional health, to determine eligibility for IRIS and TMG services.
  2. Develop care plans: Collaborate with individuals and their families to develop personalized care plans that address their specific needs and goals.
  3. Educate and inform: Provide education and guidance to individuals and their families about IRIS and TMG processes, services, and resources available to them.
  4. Facilitate access to services: Assist individuals in accessing and connecting with necessary services, such as healthcare providers, community resources, and support networks.
  5. Monitor progress: Regularly monitor and evaluate the effectiveness of care plans and make necessary adjustments to ensure the best possible outcomes for individuals.
  6. Ensure compliance: Adhere to all state and federal regulations and guidelines related to IRIS and TMG processes and documentation.
  7. Maintain records: Accurately document all interactions, assessments, and services provided to individuals in a timely and organized manner.
  8. Collaborate with team members: Work closely with other IRIS Consultants, healthcare providers, and community partners to coordinate and deliver comprehensive care to individuals.
  9. Advocate for individuals: Serve as an advocate for individuals and their families, ensuring their rights are respected and their needs are met.
  10. Stay updated: Stay informed about changes and updates in IRIS and TMG processes and procedures, and continuously expand knowledge and skills through training and development opportunities.

Job Qualifications
  • Strong Communication And Interpersonal Skills.

  • Bachelor's Degree In Social Work, Psychology, Sociology, Or Other Related Field.

  • Minimum Of 2 Years Experience In Case Management Or Care Coordination.

  • Knowledge Of Medicaid And Medicare Programs And Regulations.

  • Ability To Work Independently And Make Sound Decisions Based On Individual's Needs And Goals.
