
IRIS Consultant - TMG

- Provide support and guidance to members with developmental disabilities enrolled in the IRIS program.
- Conduct assessments and develop individualized care plans for members to address their specific needs.
- Collaborate with community resources and service providers to coordinate and implement necessary services for members.
- Serve as a liaison between members, their families, and various healthcare professionals to ensure continuity of care.
- Maintain accurate and timely documentation of member interactions, services provided, and progress towards goals.
- Educate members and their families on available resources and services to enhance their quality of life.
- Stay up-to-date on the latest developments and changes in the IRIS program and ensure compliance with all regulations and guidelines.
- Build and maintain positive relationships with members, their families, and healthcare professionals.
- Advocate for the needs and rights of members and ensure they are receiving the best possible care.
- Participate in team meetings and trainings to enhance knowledge and skills related to the position.
- Collaborate with other members of the Molina Healthcare team to provide comprehensive care to members.
- Demonstrate a commitment to Molina Healthcare's mission and values in all interactions with members and colleagues.
- Maintain confidentiality of all member information and adhere to HIPAA regulations.
- Travel to various locations to meet with members, as needed.
- Perform other duties as assigned by management to support the goals and objectives of the organization.
Bachelor's Degree In Social Work, Psychology, Counseling, Or Related Field.
Minimum Of 2 Years Of Experience Working With Individuals With Intellectual And Developmental Disabilities (Idd).
Knowledge And Understanding Of Medicaid And Medicare Programs, As Well As State-Specific Rules And Regulations.
Strong Communication And Interpersonal Skills, With The Ability To Build Rapport And Collaborate With Diverse Populations.
Proficiency In Case Management And Documentation, With The Ability To Effectively Manage A Caseload And Meet Deadlines.
