
IRIS Consultant

- Conduct comprehensive assessments: Perform thorough assessments of individuals with disabilities to determine their needs, strengths, and goals for independent living.
- Develop individualized care plans: Utilize assessment findings to create personalized care plans that address the unique needs and goals of each individual.
- Coordinate and connect with resources: Identify and connect individuals with disabilities to appropriate resources and services, such as transportation, housing, healthcare, and social support.
- Advocate for individuals: Serve as an advocate for individuals with disabilities and ensure that their rights and preferences are respected and upheld.
- Educate and inform: Provide individuals with disabilities and their families with information and education on available resources and services, as well as their rights and options for long-term care.
- Maintain accurate records: Keep detailed and accurate records of assessments, care plans, and services provided to ensure compliance with regulations and guidelines.
- Collaborate with interdisciplinary team: Work closely with other healthcare professionals, social workers, and community organizations to coordinate and enhance the care and support of individuals with disabilities.
- Monitor progress and make recommendations: Regularly monitor the progress of individuals and make recommendations for adjustments to their care plans as needed.
- Provide guidance and support: Offer guidance and support to individuals and their families as they navigate the complex healthcare system and make decisions about their care.
- Stay updated on industry developments: Continuously stay updated on changes and developments in the long-term care services industry to ensure the most current and effective support for individuals with disabilities.
Bachelor's Degree In Social Work, Psychology, Or A Related Field.
Minimum Of 2 Years Of Experience Working With Individuals With Disabilities Or Chronic Illnesses.
Knowledge Of State-Specific Medicaid And Long-Term Care Programs, Including Iris (Include, Respect, I Self-Direct).
Strong Communication And Interpersonal Skills, With The Ability To Build Rapport With Diverse Populations.
Ability To Work Independently And Manage A Caseload, While Adhering To Program Guidelines And Regulations.
