  1. Assist individuals with disabilities and older adults in managing their long-term care services and supports.
  2. Develop and maintain positive relationships with clients and their families.
  3. Educate clients on the IRIS program and its services.
  4. Conduct needs assessments and create individualized care plans.
  5. Coordinate and monitor the delivery of services to ensure quality and efficiency.
  6. Advocate for clients' rights and needs.
  7. Maintain accurate and up-to-date documentation and case files.
  8. Collaborate with other healthcare professionals and community resources to ensure comprehensive care for clients.
  9. Provide guidance and support to clients in navigating the healthcare system.
  10. Identify and address any barriers to independent living and develop solutions.
  11. Remain knowledgeable about changes and updates to the IRIS program.
  12. Adhere to all ethical and legal standards in the provision of services.
  13. Communicate effectively with clients, their families, and other team members.
  14. Continuously evaluate and monitor the effectiveness of services and make necessary adjustments.
  15. Participate in training and professional development opportunities to enhance skills and knowledge.
  16. Act as a positive representative of Molina Healthcare and uphold its values and mission.
  17. Comply with all company policies and procedures.
  18. Prioritize and manage a caseload effectively.
  19. Demonstrate compassion, empathy, and respect for all clients.
  20. Make a positive impact on the lives of clients by promoting independence and self-advocacy.

Job Qualifications
  • Bachelor's Degree In Social Work, Psychology, Counseling Or Related Field.

  • Minimum Of 2 Years Of Experience In The Disability Field, Preferably Working With Individuals With Developmental Disabilities.

  • Certification As An Iris Consultant Through The Wisconsin Department Of Health Services.

  • Knowledge And Understanding Of Wisconsin's Iris Program And Its Policies And Procedures.

  • Strong Communication And Interpersonal Skills, With The Ability To Build And Maintain Relationships With Individuals, Families, And Community Resources.
