As an IRIS Consultant, you will play a crucial role in coordinating and managing the care of our members with Intellectual and Developmental Disabilities (IDD).   1. Coordinate and manage the care of our members with Intellectual and Developmental Disabilities (IDD) by conducting comprehensive assessments, developing individualized care plans, and monitoring progress.
  2. Serve as the primary point of contact for our IDD members and their families, providing support, guidance, and resources to ensure their healthcare needs are met.
  3. Collaborate with healthcare providers, community resources, and other stakeholders to ensure the delivery of high-quality, person-centered care.
  4. Stay up-to-date on the latest developments and best practices in the field of IDD services to ensure our members receive the best possible care.
  5. Conduct regular home visits and attend meetings with members and their families to assess their needs and address any concerns or issues that may arise.
  6. Maintain accurate and timely documentation of all member interactions, care plans, and progress reports.
  7. Actively participate in training and professional development opportunities to enhance knowledge and skills related to IDD services.
  8. Build and maintain strong relationships with members, their families, and other stakeholders to ensure a positive and trusting rapport.
  9. Monitor and track member outcomes and make necessary adjustments to care plans to achieve desired results.
  10. Provide exceptional customer service by responding promptly and effectively to member and family inquiries and concerns.
  11. Adhere to all state and federal regulations, as well as company policies and procedures, to ensure compliance and quality of care.
  12. Collaborate with other members of the healthcare team to ensure seamless coordination of care and promote a multidisciplinary approach to addressing member needs.
  13. Foster a culture of inclusivity, respect, and empathy for members with IDD and their families.
  14. Act as an advocate for our members, ensuring their rights and preferences are respected and their voices are heard.

Job Qualifications
  • Bachelor's Or Master's Degree In Social Work, Psychology, Counseling, Or Related Field.

  • Minimum Of 2 Years Experience Working With Individuals With Intellectual And Developmental Disabilities.

  • Knowledge Of State Medicaid And Medicare Programs And Regulations Related To Intellectual And Developmental Disabilities Services.

  • Strong Communication And Interpersonal Skills To Build Rapport With Clients And Their Families.

  • Ability To Develop And Implement Individualized Service Plans And Monitor Progress Towards Goals.
