  1. Coordinate and manage care for patients with chronic conditions, ensuring they receive the best quality of care.
  2. Develop and implement individualized care plans for members in need, in collaboration with the healthcare team.
  3. Conduct regular assessments of members' health needs and make appropriate recommendations for care and services.
  4. Monitor and track members' progress in meeting their health goals and make adjustments to care plans as needed.
  5. Communicate effectively with members, their families, and healthcare providers to ensure coordinated and comprehensive care.
  6. Collaborate with community resources and agencies to connect members with additional support and services.
  7. Document and maintain accurate records of members' care plans, progress, and any changes in their health status.
  8. Serve as an advocate for members, ensuring they receive timely and appropriate care from all healthcare providers.
  9. Educate members and their families on their health conditions, treatment options, and self-management techniques.
  10. Stay updated on current healthcare policies, procedures, and regulations, and ensure compliance with all applicable standards.
  11. Build and maintain relationships with members from diverse backgrounds, demonstrating cultural competency and sensitivity.
  12. Participate in team meetings and provide valuable input on improving the quality of care and services provided to members.
  13. Continuously evaluate and improve care coordination processes to enhance the overall health and well-being of members.
  14. Maintain confidentiality of members' personal and health information in accordance with HIPAA regulations.
  15. Collaborate with other healthcare professionals and departments within Molina Healthcare to provide integrated and coordinated care to members.
