- Provide case management services to members enrolled in Molina Healthcare's LTSS program.
- Assess member needs and develop individualized care plans to help them access and coordinate long-term support services.
- Collaborate with healthcare providers and community resources to ensure members receive appropriate and timely care.
- Conduct home visits and follow-up appointments to monitor member progress and address any concerns.
- Communicate effectively with members, their families, and healthcare team to ensure coordinated and comprehensive care.
- Maintain accurate and up-to-date documentation of all interactions and interventions with members.
- Advocate for members' needs and rights, ensuring they receive the best possible care.
- Identify and address any barriers to care and work to resolve them.
- Participate in training and development opportunities to stay current on industry trends and best practices.
- Adhere to all company policies and procedures, as well as state and federal regulations.
- Maintain confidentiality of all member information in accordance with HIPAA and company policies.
- Collaborate with other members of the healthcare team to improve the overall health and well-being of members.
- Travel to members' homes, facilities, and community locations as needed.
- Maintain a positive and professional attitude while representing Molina Healthcare.
- Act as a resource for members and their families, providing education and support.
- Attend meetings and conferences as required.
- Meet productivity and performance standards set by the company.
- Comply with all safety and infection control policies and procedures.
- Continuously strive to improve the quality of care and services provided to members.

Bachelor's Degree In Social Work, Nursing, Or Related Field: A Strong Educational Background In A Relevant Field Is Essential For A Field Case Manager, Ltss Position At Molina Healthcare. This Ensures That The Candidate Has A Solid Foundation Of Knowledge And Skills To Effectively Manage And Coordinate Care For Patients.
Experience In Long-Term Services And Supports: Previous Experience In Providing Case Management Services For Individuals Receiving Long-Term Services And Supports Is A Necessary Qualification For This Role.
