  1. Provide case management services to members enrolled in Molina Healthcare's LTSS program.
  2. Assess member needs and develop individualized care plans to help them access and coordinate long-term support services.
  3. Collaborate with healthcare providers and community resources to ensure members receive appropriate and timely care.
  4. Conduct home visits and follow-up appointments to monitor member progress and address any concerns.
  5. Communicate effectively with members, their families, and healthcare team to ensure coordinated and comprehensive care.
  6. Maintain accurate and up-to-date documentation of all interactions and interventions with members.
  7. Advocate for members' needs and rights, ensuring they receive the best possible care.
  8. Identify and address any barriers to care and work to resolve them.
  9. Participate in training and development opportunities to stay current on industry trends and best practices.
  10. Adhere to all company policies and procedures, as well as state and federal regulations.
  11. Maintain confidentiality of all member information in accordance with HIPAA and company policies.
  12. Collaborate with other members of the healthcare team to improve the overall health and well-being of members.
  13. Travel to members' homes, facilities, and community locations as needed.
  14. Maintain a positive and professional attitude while representing Molina Healthcare.
  15. Act as a resource for members and their families, providing education and support.
  16. Attend meetings and conferences as required.
  17. Meet productivity and performance standards set by the company.
  18. Comply with all safety and infection control policies and procedures.
  19. Continuously strive to improve the quality of care and services provided to members.
  20. Other duties as assigned by supervisor.

Job Qualifications
  • Bachelor's Degree In Social Work, Nursing, Or Related Field: A Strong Educational Background In A Relevant Field Is Essential For A Field Case Manager, Ltss Position At Molina Healthcare. This Ensures That The Candidate Has A Solid Foundation Of Knowledge And Skills To Effectively Manage And Coordinate Care For Patients.

  • Experience In Long-Term Services And Supports: Previous Experience In Providing Case Management Services For Individuals Receiving Long-Term Services And Supports Is A Necessary Qualification For This Role. This Demonstrates The Candidate's Understanding Of The Unique Needs And Challenges Of This Population.

  • Knowledge Of Medicaid And Medicare Programs: A Strong Understanding Of Medicaid And Medicare Programs Is Crucial For A Field Case Manager, Ltss At Molina Healthcare. This Includes Knowledge Of Eligibility Requirements, Coverage And Benefits, And Regulations Related To These Programs.

  • Excellent Communication And Interpersonal Skills: As A Field Case Manager, Ltss, The Candidate Will Be Working Closely With Patients, Their Families, Healthcare Providers, And Other Stakeholders. Strong Communication And Interpersonal Skills Are Essential For Building And Maintaining Positive Relationships With These Individuals.

  • Case Management Certification: Having A Certification In Case Management, Such As The Certified Case Manager (Ccm) Credential, Is Highly Desirable For This Position. This Demonstrates The Candidate's Commitment To The Field And Their Proficiency In Providing High-Quality Case Management Services.
