- Coordinate and manage the care for our members who require long-term services and supports.
- Develop and implement individualized care plans for each member.
- Conduct in-person visits to assess members' needs and ensure they are receiving appropriate services.
- Collaborate with healthcare providers, community resources, and other stakeholders to ensure comprehensive and quality care for members.
- Monitor and track members' progress and make necessary adjustments to their care plans.
- Advocate for members' needs and ensure they receive timely and appropriate services.
- Provide education and support to members and their families regarding available resources and services.
- Maintain accurate and up-to-date documentation of all member interactions and care plans.
- Adhere to all regulatory and organizational guidelines and standards.
- Serve as a liaison between members, providers, and other healthcare professionals.
- Keep abreast of industry updates, changes, and trends related to long-term services and supports.
- Collaborate with other members of the healthcare team to improve overall member outcomes.
- Participate in regular team meetings and case conferences to discuss member progress and identify areas for improvement.
- Demonstrate compassion, empathy, and understanding towards members and their families.
- Continuously strive to improve the quality and effectiveness of care provided to members.
Bachelor's Degree In Social Work, Nursing, Or Related Field: A Field Case Manager, Ltss At Molina Healthcare Should Have A Strong Educational Background In A Field Related To Healthcare Or Social Work. This Provides A Foundation For Understanding The Complex Needs Of Ltss Members And Navigating The Healthcare System.
Experience In Healthcare Case Management: Applicants For This Role Should Have A Minimum Of 2-3 Years Of Experience In Healthcare Case Management, Preferably In A Managed Care Setting. This Experience Demonstrates An Understanding Of The Role And Responsibilities Of A Case Manager And The Ability To Effectively Manage A Caseload.
Knowledge Of Ltss Programs And Regulations: Familiarity With Ltss Programs, Including Medicaid And Medicare, As Well As Regulations And Guidelines Related To These Programs, Is Essential For A Field Case Manager, Ltss At Molina Healthcare. This Knowledge Allows The Case Manager To Effectively Advocate For Members And Ensure Compliance With Program Requirements.
Strong Communication And Interpersonal Skills: As A Field Case Manager, Ltss, Effective Communication And Interpersonal Skills Are Crucial For Building Relationships With Members, Caregivers, And Healthcare Providers. The Ability To Communicate Clearly And Compassionately Is Essential For Providing Quality Care And Support To Members.
Ability To Work Independently And In A Team: A Successful Field Case Manager, Ltss Should Be Able To Work Independently, Managing Their Own Schedule And Caseload. However, They Should Also Be Able To Collaborate And Work Effectively With A Multidisciplinary Team, Including Nurses, Social Workers, And Other Healthcare Professionals, To Provide Comprehensive Care To Members.
