  1. Conduct in-depth assessments of each member's needs, including medical, social, and environmental factors.
  2. Develop and implement personalized care plans that address the unique needs of each member.
  3. Monitor and evaluate the effectiveness of care plans, making necessary adjustments to ensure the best outcomes for members.
  4. Collaborate with interdisciplinary teams to coordinate and facilitate the delivery of services to members.
  5. Advocate for members and ensure they receive appropriate and timely care.
  6. Educate members on their healthcare options and empower them to make informed decisions about their health.
  7. Maintain accurate and up-to-date documentation of all case management activities.
  8. Keep abreast of changes in LTSS programs and regulations and ensure compliance with all applicable policies.
  9. Participate in continuous quality improvement initiatives to enhance the overall effectiveness of the LTSS program.
  10. Communicate regularly with members, caregivers, and healthcare providers to ensure seamless coordination of care.
  11. Provide support and guidance to members and their families during times of crisis or difficult transitions.
  12. Act as a liaison between members and community resources to address any social or environmental factors that may impact their health.
  13. Collaborate with other members of the healthcare team to ensure a holistic and integrated approach to care.
  14. Demonstrate compassion, empathy, and professionalism in all interactions with members and their families.
  15. Uphold Molina Healthcare's mission and values in all aspects of the job.

Job Qualifications
  • Bachelor's Degree In Social Work, Nursing, Or A Related Field.

  • Minimum Of 3-5 Years Experience In Case Management, Preferably In The Ltss (Long-Term Services And Supports) Field.

  • Knowledge Of Medicare And Medicaid Regulations And Policies Related To Ltss.

  • Strong Communication And Interpersonal Skills, With The Ability To Build Relationships With Clients, Providers, And Community Resources.

  • Ability To Work Independently And Manage A Caseload While Adhering To Deadlines And Completing Required Documentation Accurately.
