Job Description

Are you a compassionate and dedicated RN with experience in long-term care? Do you have a strong understanding of LTSS programs and a passion for providing high-quality care to patients? If so, Molina Healthcare has an exciting opportunity for you as a Field Care Manager, LTSS. In this role, you will have the opportunity to make a meaningful impact on the lives of our members by coordinating and managing their long-term care needs.   1. Coordinate and manage the long-term care needs of Molina Healthcare members.
  2. Develop and implement individualized care plans for members, collaborating with other healthcare professionals as needed.
  3. Conduct comprehensive assessments of member's health, social, and functional needs.
  4. Monitor and evaluate the effectiveness of care plans and make necessary adjustments.
  5. Educate members and their families on available LTSS programs and resources.
  6. Advocate for members to ensure they receive appropriate and timely care.
  7. Communicate effectively with members, their families, and healthcare providers to ensure continuity of care.
  8. Document all interactions, assessments, and care plans accurately and in a timely manner.
  9. Stay current on industry changes, best practices, and regulations related to LTSS programs.
  10. Collaborate with interdisciplinary team members to ensure seamless coordination of care.
  11. Participate in quality improvement initiatives to enhance the delivery of care.
  12. Assist with discharge planning and transition to lower levels of care, as needed.
  13. Maintain confidentiality and adhere to all HIPAA regulations.
  14. Remain proactive in identifying and addressing potential barriers to care.
  15. Demonstrate compassion and empathy while providing care to members.
  16. Uphold Molina Healthcare's mission, vision, and values in all interactions.
  17. Attend training and development programs to enhance knowledge and skills.
  18. Adhere to all company policies and procedures.
  19. Engage in open communication and collaboration with colleagues and supervisors.
  20. Continuously strive to provide high-quality care and improve member outcomes.

Job Qualifications
  • Active Rn License In The State Of Employment.

  • Strong Organizational And Time Management Skills, With The Ability To Manage A Caseload And Prioritize Tasks Effectively.

  • Minimum Of 3 Years Of Clinical Experience In A Healthcare Setting, Preferably In Long-Term Care Or Case Management.

  • Knowledge And Understanding Of Ltss Programs And Regulations, Including Medicare And Medicaid.

  • Excellent Communication And Interpersonal Skills, With The Ability To Effectively Collaborate With Interdisciplinary Teams And Communicate Complex Medical Information To Patients And Caregivers.
