- Serve as a liaison between Molina Healthcare and the communities we serve.
- Develop and maintain strong relationships with community partners and stakeholders.
- Identify healthcare needs within the community and work to address them.
- Educate community members about available healthcare resources and services.
- Collaborate with internal teams to develop and implement community outreach programs.
- Plan and organize outreach events, such as health fairs and workshops.
- Conduct outreach to underserved populations and assist them with accessing healthcare services.
- Keep up-to-date with changes and updates in the healthcare industry to better serve the community.
- Utilize strong communication skills to effectively communicate with community members and partners.
- Maintain accurate records and documentation of outreach activities and community interactions.
- Collaborate with other Community Connectors to ensure consistent and effective outreach efforts.
- Represent Molina Healthcare at community events and meetings.
- Adhere to all regulations and guidelines related to community outreach and healthcare.
- Use data and analytics to track the impact of outreach efforts and make recommendations for improvement.
- Continuously assess the needs and concerns of the community and provide feedback to Molina Healthcare.
- Maintain a positive and professional attitude while working with community members and partners.
- Adhere to all company policies and procedures.
- Attend trainings and professional development opportunities to enhance skills and knowledge.
- Travel to various locations within the community to conduct outreach and attend meetings.
- Act as a positive ambassador for Molina Healthcare and its mission to improve the health and well-being of the community.
Excellent Communication And Interpersonal Skills.
Bachelor's Degree In Social Work, Public Health, Or A Related Field.
Minimum Of 2 Years Of Experience In Community Outreach Or Engagement.
Strong Knowledge Of Community Resources And Social Services.
Ability To Work Independently And Collaboratively With Diverse Populations.
