Your strong communication and organizational skills, coupled with your knowledge of healthcare systems, will make you an invaluable asset to our team and the communities we serve.To be successful in this role, you must have a high school diploma or equivalent, and at least two years of experience in healthcare or community outreach.   1. Serve as a liaison between Molina Healthcare and the communities we serve.
  2. Develop and maintain strong relationships with community partners and stakeholders.
  3. Identify healthcare needs within the community and work to address them.
  4. Educate community members about available healthcare resources and services.
  5. Collaborate with internal teams to develop and implement community outreach programs.
  6. Plan and organize outreach events, such as health fairs and workshops.
  7. Conduct outreach to underserved populations and assist them with accessing healthcare services.
  8. Keep up-to-date with changes and updates in the healthcare industry to better serve the community.
  9. Utilize strong communication skills to effectively communicate with community members and partners.
  10. Maintain accurate records and documentation of outreach activities and community interactions.
  11. Collaborate with other Community Connectors to ensure consistent and effective outreach efforts.
  12. Represent Molina Healthcare at community events and meetings.
  13. Adhere to all regulations and guidelines related to community outreach and healthcare.
  14. Use data and analytics to track the impact of outreach efforts and make recommendations for improvement.
  15. Continuously assess the needs and concerns of the community and provide feedback to Molina Healthcare.
  16. Maintain a positive and professional attitude while working with community members and partners.
  17. Adhere to all company policies and procedures.
  18. Attend trainings and professional development opportunities to enhance skills and knowledge.
  19. Travel to various locations within the community to conduct outreach and attend meetings.
  20. Act as a positive ambassador for Molina Healthcare and its mission to improve the health and well-being of the community.

Job Qualifications
