- Build and maintain strong relationships within the community to identify and connect individuals with necessary resources and support services.
- Educate community members, including Molina Healthcare members, about available services and how to access them.
- Collaborate with community organizations, healthcare providers, and social service agencies to develop and implement outreach strategies.
- Conduct outreach efforts to engage and enroll eligible individuals into Molina Healthcare programs.
- Provide support and assistance to individuals in navigating and accessing healthcare services.
- Conduct home visits and participate in community events to connect with community members and promote Molina Healthcare services.
- Maintain accurate and organized records of community engagement activities and member outreach efforts.
- Continuously assess and monitor the needs of the community to identify potential gaps in services and resources.
- Serve as a liaison between community members and Molina Healthcare, addressing any concerns or issues that may arise.
- Stay up-to-date on community resources and social services to effectively connect individuals with appropriate support.
- Collaborate with internal teams to ensure coordinated and comprehensive care for community members.
- Adhere to all company policies and procedures, as well as state and federal regulations, in all aspects of the role.
Bachelor's Degree In Social Work, Public Health, Or A Related Field
Minimum Of 2 Years Of Experience In Community Outreach Or Healthcare Coordination
Strong Understanding Of Healthcare Systems And Resources Available To Underserved Communities
Excellent Communication And Interpersonal Skills For Building And Maintaining Relationships With Community Partners And Members
Bilingual Proficiency In English And Another Language Commonly Spoken In The Local Community.
