10. Stay up-to-date on community resources and social services to effectively connect individuals with appropriate support.
11. Collaborate with internal teams to ensure coordinated and comprehensive care for community members.
12. Adhere to all company policies and procedures, as well as state and federal regulations, in all aspects of the role.
