Job Description

Molina Healthcare is looking for an experienced and passionate Registered Nurse (RN) to join our team as a Transplant Case Manager. In this position, you will be responsible for providing case management services to patients, families, and caregivers in support of the transplant program. You will coordinate patient care across the continuum of care, ensuring effective, efficient, and timely care.To be successful in this role, you must have exceptional organizational and interpersonal skills, as well as a strong understanding of the transplant process. You must have a current nursing license, at least 1 year of case management experience, and a Bachelor's degree in Nursing. Additionally, you must have the ability to work autonomously, as well as collaborate with other healthcare professionals. A passion for delivering quality care and helping others is essential for success in this role.
