  1. Develop and implement comprehensive care plans for members in need of long-term support services.
  2. Conduct thorough assessments of members' physical, mental, and emotional needs.
  3. Collaborate with healthcare providers and community resources to coordinate and facilitate appropriate care for members.
  4. Advocate for members to ensure they receive necessary services and support.
  5. Monitor and evaluate the effectiveness of care plans and make necessary adjustments.
  6. Maintain accurate and up-to-date documentation of member interactions and progress.
  7. Educate members and their families on available resources and support services.
  8. Communicate effectively with members, families, and healthcare team members to ensure continuity of care.
  9. Stay current on best practices and regulations related to long-term support services.
  10. Adhere to all privacy and confidentiality policies and procedures.
  11. Participate in interdisciplinary team meetings to discuss member care and collaborate on solutions.
  12. Provide emotional support and guidance to members and their families.
  13. Identify and address any potential barriers to care and develop strategies to overcome them.
  14. Serve as a liaison between members, healthcare providers, and community organizations.
  15. Promote a positive and patient-centered work environment.

Job Qualifications
  • Registered Nurse (Rn) License: A Case Manager, Ltss At Molina Healthcare Must Hold A Current And Active Rn License In The State Where They Are Practicing.

  • Experience In Long-Term Services And Supports (Ltss): Candidates Should Have A Minimum Of 2-3 Years Of Experience Working In The Ltss Field, With A Strong Understanding Of Medicaid And Medicare Programs.

  • Case Management Experience: Previous Experience In Case Management, Preferably In A Healthcare Setting, Is Required. This Includes Experience With Care Coordination, Utilization Management, And Discharge Planning.

  • Knowledge Of Managed Care: Familiarity With Managed Care Principles And Practices, As Well As Knowledge Of State And Federal Regulations Related To Ltss, Is Highly Preferred.

  • Excellent Communication And Interpersonal Skills: A Case Manager, Ltss Must Have Strong Communication Skills, Both Written And Verbal, And The Ability To Build Relationships With Members, Providers, And Other Healthcare Professionals. This Role Also Requires Strong Organizational And Problem-Solving Skills.
