1. Coordinate and manage long-term services and supports for members to ensure their healthcare needs are met.
2. Develop individualized care plans based on member assessments and collaborate with interdisciplinary teams to implement and monitor them.
3. Monitor and evaluate the effectiveness and appropriateness of care plans, making adjustments as needed.
4. Provide ongoing support and guidance to members and their families, connecting them with necessary resources and services.
5. Conduct regular home visits and assessments to ensure the safety and well-being of members.
6. Communicate and collaborate with healthcare providers, community resources, and other stakeholders to advocate for members and ensure continuity of care.
7. Maintain accurate and timely documentation of member interactions, care plans, and progress.
8. Adhere to all regulatory and compliance guidelines in the delivery of care.
9. Participate in staff meetings, trainings, and continuing education opportunities to stay current on best practices and industry updates.
