- Coordinate and manage the long-term care services and supports for our members.
- Conduct assessments and develop individualized care plans for members.
- Collaborate with healthcare providers and community resources to ensure comprehensive care for members.
- Monitor and evaluate the effectiveness of care plans and make necessary adjustments.
- Educate members and their families on available resources and benefits.
- Advocate for members' rights and ensure they receive appropriate care and services.
- Maintain accurate and up-to-date documentation of member interactions and care plans.
- Communicate and collaborate with interdisciplinary team members to ensure continuity of care.
- Identify and address any barriers to accessing care for members.
- Stay current on industry trends and best practices in long-term services and supports.
- Participate in quality improvement initiatives and ensure compliance with regulatory standards.
- Serve as a resource for colleagues and provide training and guidance on LTSS.

Strong Communication And Interpersonal Skills
Knowledge Of Medicaid And Medicare Regulations And Guidelines
Bachelor's Degree In Social Work, Healthcare Management, Or Related Field
Minimum Of 2 Years Of Experience In Case Management Or Long-Term Services And Supports (Ltss) Coordination
