
Case Manager Behavioral Health

- Conduct initial assessments and ongoing evaluations to determine the mental health needs of each member.
- Develop and implement individualized care plans for each member, in collaboration with the member, their family, and their healthcare team.
- Monitor and track member progress and adjust care plans as needed.
- Provide education and support to members and their families about mental health conditions, treatment options, and community resources.
- Advocate for members' needs and rights, including coordinating with external agencies and providers.
- Collaborate with healthcare providers to ensure integrated and coordinated care for members.
- Document all interactions and interventions accurately and in a timely manner.
- Conduct regular follow-up with members to ensure continuity of care and address any barriers to treatment.
- Participate in multidisciplinary team meetings and case conferences to discuss member progress and develop strategies for improving outcomes.
- Stay updated on current mental health treatment modalities and evidence-based practices.
- Adhere to all ethical and legal standards in the delivery of case management services.
- Maintain confidentiality of all member information and records.
- Communicate effectively with members, their families, and healthcare team members to ensure a collaborative and patient-centered approach to care.
- Act as a resource for members and their families, providing information and guidance on mental health resources and services.
- Participate in quality improvement initiatives and contribute to the development of best practices for mental health case management.
Bachelor's Or Master's Degree In Social Work, Psychology, Counseling, Or A Related Field.
Minimum Of 2-3 Years Of Experience In Behavioral Health Case Management, Preferably In A Managed Care Setting.
Knowledge Of Mental Health And Substance Abuse Treatment Modalities, As Well As Community Resources And Support Services.
Excellent Communication, Interpersonal, And Organizational Skills.
Ability To Work Independently And Collaboratively With A Multidisciplinary Team To Provide Comprehensive Care Coordination And Support To Individuals With Complex Behavioral Health Needs.
