As a Case Management Processor in our Behavioral Health department, you will be responsible for coordinating and managing the care of our members, ensuring they receive the support and resources they need to improve their overall well-being.   1. Coordinate and manage the care of members struggling with behavioral health challenges.
  2. Ensure that members receive the necessary support and resources to improve their overall well-being.
  3. Understand and apply behavioral health principles to effectively address the needs of members.
  4. Communicate with members, their families, and healthcare providers to develop and implement individualized care plans.
  5. Monitor and evaluate the progress of members and make adjustments to care plans as needed.
  6. Maintain accurate and up-to-date documentation of member interactions and progress.
  7. Collaborate with internal and external stakeholders to ensure continuity of care for members.
  8. Adhere to all regulatory and organizational guidelines and policies.
  9. Demonstrate empathy and compassion towards members while maintaining professional boundaries.
  10. Stay current on industry developments and best practices in behavioral health to continuously improve the quality of care provided.
  11. Prioritize and manage a caseload to effectively meet the needs of members.
  12. Handle confidential information with discretion and follow all HIPAA regulations.
  13. Participate in team meetings and trainings to enhance skills and knowledge.
  14. Proactively identify opportunities for process improvement and offer suggestions for implementation.
  15. Uphold Molina Healthcare's mission and values in all interactions and work responsibilities.

Job Qualifications
  • Bachelor's Degree In Social Work, Psychology, Or A Related Field.

  • Minimum Of 2 Years Of Experience In Case Management Or Behavioral Health Services.

  • Knowledge Of Mental Health And Substance Abuse Treatment Principles And Resources.

  • Strong Communication And Interpersonal Skills To Effectively Work With Clients, Providers, And Other Team Members.

  • Ability To Effectively Document And Maintain Case Records In Compliance With Regulatory Standards.
