Job Description

As a Care Manager, you will play a crucial role in coordinating care for our members, ensuring they receive the necessary services and resources to improve their overall well-being.   1. Manage and coordinate the delivery of long-term services and supports (LTSS) to Molina Healthcare members.
  2. Develop and maintain relationships with members, families, caregivers, and other healthcare professionals to ensure effective communication and collaboration.
  3. Conduct comprehensive assessments of member needs and create individualized care plans to address those needs.
  4. Monitor and track the progress of members and adjust care plans as needed to ensure optimal outcomes.
  5. Advocate for members' rights and needs, and ensure they have access to all necessary services and resources.
  6. Collaborate with interdisciplinary teams to facilitate and coordinate care, including medical, behavioral health, and social services.
  7. Ensure that all care provided is in compliance with regulatory and organizational standards.
  8. Conduct regular home visits and attend care team meetings to assess and address member needs.
  9. Document all care management activities accurately and in a timely manner.
  10. Stay up-to-date on best practices and industry standards in LTSS and integrate them into care management processes.
  11. Participate in training and professional development opportunities to enhance knowledge and skills.
  12. Adhere to confidentiality and privacy policies when handling sensitive member information.
  13. Communicate effectively with members, families, and healthcare professionals to provide education and support regarding LTSS.
  14. Collaborate with community resources and organizations to connect members with additional support and services.
  15. Demonstrate a commitment to Molina Healthcare's mission and values, and promote a culture of high-quality, compassionate care.

Job Qualifications
  • Bachelor's Degree In A Related Field Such As Nursing, Social Work, Or Healthcare Administration.

  • Knowledge And Understanding Of Ltss Programs And Regulations, Including Medicaid And Medicare.

  • Minimum Of 3 Years Of Experience In Healthcare Management, Preferably In Long-Term Services And Supports (Ltss).

  • Strong Communication And Interpersonal Skills, With The Ability To Effectively Collaborate With Diverse Teams And Stakeholders.

  • Experience In Care Coordination And Case Management, With A Focus On Improving Health Outcomes And Managing Complex Medical And Social Needs.
