- Manage and coordinate the delivery of long-term services and supports (LTSS) to Molina Healthcare members.
- Develop and maintain relationships with members, families, caregivers, and other healthcare professionals to ensure effective communication and collaboration.
- Conduct comprehensive assessments of member needs and create individualized care plans to address those needs.
- Monitor and track the progress of members and adjust care plans as needed to ensure optimal outcomes.
- Advocate for members' rights and needs, and ensure they have access to all necessary services and resources.
- Collaborate with interdisciplinary teams to facilitate and coordinate care, including medical, behavioral health, and social services.
- Ensure that all care provided is in compliance with regulatory and organizational standards.
- Conduct regular home visits and attend care team meetings to assess and address member needs.
- Document all care management activities accurately and in a timely manner.
- Stay up-to-date on best practices and industry standards in LTSS and integrate them into care management processes.
- Participate in training and professional development opportunities to enhance knowledge and skills.
- Adhere to confidentiality and privacy policies when handling sensitive member information.
- Communicate effectively with members, families, and healthcare professionals to provide education and support regarding LTSS.
- Collaborate with community resources and organizations to connect members with additional support and services.
- Demonstrate a commitment to Molina Healthcare's mission and values, and promote a culture of high-quality, compassionate care.
Bachelor's Degree In A Related Field Such As Nursing, Social Work, Or Healthcare Administration.
Knowledge And Understanding Of Ltss Programs And Regulations, Including Medicaid And Medicare.
Minimum Of 3 Years Of Experience In Healthcare Management, Preferably In Long-Term Services And Supports (Ltss).
Strong Communication And Interpersonal Skills, With The Ability To Effectively Collaborate With Diverse Teams And Stakeholders.
Experience In Care Coordination And Case Management, With A Focus On Improving Health Outcomes And Managing Complex Medical And Social Needs.
