  1. Coordinate and manage the care of Molina Healthcare members in the state of Illinois.
  2. Ensure members receive necessary services and resources to improve their health and well-being.
  3. Work closely with healthcare providers and community resources to develop and implement care plans for members.
  4. Maintain accurate and up-to-date records of member care and progress.
  5. Communicate effectively with members, their families, and healthcare providers to ensure a collaborative and comprehensive approach to care.
  6. Review and analyze member health data to identify areas for improvement and develop strategies to address them.
  7. Collaborate with other members of the care management team, including nurses and social workers, to provide holistic care to members.
  8. Adhere to all applicable regulations and guidelines related to healthcare and member confidentiality.
  9. Participate in training and development opportunities to continuously improve knowledge and skills.
  10. Demonstrate a commitment to Molina Healthcare's mission of providing quality care to our members.

Job Qualifications
  • Bachelor's Degree In Healthcare Administration Or A Related Field.

  • At Least 2 Years Of Experience In Care Management Or Utilization Management.

  • Knowledge Of Healthcare Regulations And Guidelines, Particularly In The State Of Illinois.

  • Excellent Communication And Interpersonal Skills, With The Ability To Work Effectively With Patients, Providers, And Other Healthcare Professionals.
