Social Worker - Outpatient Care Manager

In this role, you will be responsible for providing social work services to patients and families in the outpatient setting. You must have a master’s degree in social work, be a Licensed Clinical Social Worker (LCSW) in the State of Maryland, and have a minimum of three years of experience in a healthcare setting. Responsibilities: • Provide social work services to patients and families in an outpatient setting • Develop and implement individualized treatment plans for patients and families • Monitor and evaluate patient progress and adjust treatment plans as needed • Collaborate with other healthcare providers to ensure optimal patient care • Facilitate family meetings to discuss treatment plans and progress • Identify community resources and coordinate referrals as needed • Provide crisis intervention services when needed • Participate in educational and clinical research programs • Maintain timely and accurate patient records • Participate in quality improvement initiatives and program development
