Social Worker

You must also have a minimum of two years of experience in a clinical setting. The ability to work independently and collaboratively with a team is essential. Responsibilities of a Social Worker:

1. Assess patient and family needs and provide appropriate interventions.
2. Develop and implement service plans in a culturally-sensitive manner.
3. Provide individual and family counseling to address patient and family needs.
4. Establish and maintain relationships with external stakeholders.
5. Develop and implement programs and services to meet the needs of vulnerable populations.
6. Participate in community outreach activities and events to promote awareness of services.
7. Educate and advocate on behalf of patients and their families.
8. Follow up with patients and families to monitor progress and ensure services are meeting their needs.
9. Document all activities and outcomes in accordance with agency policy.
10. Maintain professional and technical knowledge by attending workshops, seminars, and conferences.
