Discharge Lounge PCT

We are currently seeking a dedicated and compassionate PCT to join our team in the Discharge Lounge. In this role, you will play a vital role in ensuring our patients have a smooth transition from our hospital to their next level of care. 1. Greet and welcome patients and their families to the Discharge Lounge in a warm and friendly manner.
2. Assist with the discharge process by preparing necessary paperwork, providing instructions, and answering any questions or concerns.
3. Monitor and assess patients' physical and emotional well-being during their stay in the Discharge Lounge.
4. Collaborate with the interdisciplinary team to develop and implement a personalized discharge plan for each patient.
5. Communicate effectively with patients and their families to ensure they have a clear understanding of their post-hospital care instructions.
6. Provide emotional support and reassurance to patients and their families during the discharge process.
7. Ensure the Discharge Lounge is clean, organized, and stocked with necessary supplies.
8. Follow infection control protocols to maintain a safe and healthy environment for patients and staff.
9. Use proper body mechanics and equipment to safely transfer and transport patients.
10. Document patient interactions and any changes in their condition accurately and in a timely manner.
11. Adhere to all hospital policies and procedures, as well as state and federal regulations.
12. Attend staff meetings and participate in ongoing education and training to maintain and improve job knowledge and skills.
13. Maintain confidentiality and uphold patient privacy at all times.
14. Serve as a positive and professional representative of MercyOne to patients, families, and visitors.
15. Perform other duties as assigned by the supervisor or manager.
