
Hospice Liaison, Registered Nurse (RN)
We are looking for an experienced and passionate Registered Nurse (RN) to join our Hospice Liaison team at MemorialCare. Our ideal candidate is an experienced RN who is committed to providing effective, compassionate, and holistic care to our hospice patients and their families. In this role, you will be responsible for coordinating and supporting patient care services for hospice patients and their families.To be successful in this role, candidates must have a valid RN license, at least two years of experience in hospice or palliative care, and strong interpersonal and communication skills. You must also be knowledgeable in evidence-based practice, regulatory requirements, and pain and symptom management. The ability to work independently and collaboratively as a member of an interdisciplinary team is also essential.
