
Nurse Navigator-Medical Oncology

- Provide personalized care and guidance to patients diagnosed with cancer.
- Serve as a point of contact and support for patients throughout their cancer journey.
- Collaborate with oncologists and other healthcare professionals to develop and implement treatment plans.
- Educate patients and their families about the disease, treatment options, and potential side effects.
- Monitor and assess patients' physical and emotional well-being, and provide appropriate interventions and referrals as needed.
- Coordinate and schedule appointments, procedures, and tests for patients.
- Advocate for patients' needs and concerns within the healthcare team.
- Maintain accurate and up-to-date patient records and documentation.
- Participate in multidisciplinary team meetings to discuss patient care and treatment plans.
- Stay current on advancements and changes in oncology treatments and care practices.
- Provide emotional support and counseling to patients and their families.
- Collaborate with community resources and organizations to provide additional support for patients.
- Serve as a liaison between patients, healthcare providers, and insurance companies.
- Ensure compliance with all regulatory and organizational guidelines.
- Continuously evaluate and improve the navigation program to meet the needs of patients and the organization.
Bachelor's Degree In Nursing (Bsn) From An Accredited Institution.
Current And Valid Rn License In The State Of Employment.
Minimum Of 3 Years Of Clinical Experience In Oncology Or Related Specialty.
Certification In Oncology Nursing (Ocn) Or Related Specialty Preferred.
Strong Communication And Interpersonal Skills, With The Ability To Effectively Educate And Support Patients And Their Families Throughout The Cancer Journey.
