  1. Serve as a primary point of contact for patients, providing guidance and support throughout their healthcare journey.
  2. Establish strong relationships with patients, families, and healthcare providers to facilitate effective communication and collaboration.
  3. Conduct thorough assessments of patients' needs and develop personalized care plans based on their individual circumstances.
  4. Coordinate and schedule appointments, procedures, and tests for patients to ensure timely and efficient care.
  5. Educate patients about their condition, treatment options, and resources available to them.
  6. Empower patients to make informed decisions about their healthcare by providing them with accurate and up-to-date information.
  7. Monitor and evaluate patient progress and outcomes, advocating for necessary interventions or adjustments to the care plan as needed.
  8. Collaborate with interdisciplinary teams to ensure continuity of care and address any barriers or challenges to patient care.
  9. Document all patient interactions and maintain accurate records to facilitate communication and continuity of care.
  10. Continuously update knowledge and skills through education and training opportunities to provide the highest quality of care to patients.

Job Qualifications
  • Bachelor's Or Master's Degree In Nursing: A Nurse Navigator At Medstar Health Must Possess A Minimum Of A Bachelor's Degree In Nursing (Bsn) Or A Master's Degree In Nursing (Msn). This Education Equips Them With The Necessary Knowledge And Skills To Provide High-Quality Patient Care.

  • Active Nursing License: The Nurse Navigator Must Hold A Current And Active Nursing License In The State Where They Will Be Working. This Ensures That They Are Qualified To Practice Nursing And Adhere To All State Regulations And Standards.

  • Minimum Of 2 Years Of Experience In A Clinical Setting: Medstar Health Requires A Nurse Navigator To Have A Minimum Of 2 Years Of Experience Working In A Clinical Setting. This Experience Provides Them With An Understanding Of Patient Care And The Ability To Navigate Complex Healthcare Systems.

  • Strong Communication And Interpersonal Skills: As A Nurse Navigator, Effective Communication Is Crucial In Building Relationships With Patients, Their Families, And Healthcare Providers. The Ability To Communicate Clearly And Empathetically Is Essential For Providing The Best Possible Care.

  • Knowledge Of Healthcare Systems And Resources: A Nurse Navigator Must Have A Thorough Understanding Of The Healthcare System And Available Resources To Assist Patients In Navigating Their Care. This Includes Knowledge Of Insurance Coverage, Medical Procedures, And Community Resources For Support And Assistance.
