
Community Health Program Coordinator
MedStar Health is looking for a passionate and dedicated Community Health Program Coordinator to join our team. The ideal candidate for this position will be a highly organized individual with an eye for detail and a commitment to making a positive impact in the lives of our patients and the surrounding community.The Community Health Program Coordinator will be responsible for developing and implementing health initiatives and programs that are dedicated to improving the well-being of our community. This position requires a strong understanding of health care and public health, and the ability to work with multiple stakeholders to ensure successful program implementation.Qualifications for this position include a Bachelor’s degree in Public Health, Health Administration, or a related field, as well as at least three years of experience in health program design, implementation, and evaluation. Additionally, the successful candidate will have excellent communication skills, both verbal and written, and the ability to work independently and collaboratively.
Responsibilities:
- Develop and implement community health initiatives and programs that focus on improving the overall health and well-being of patients and the surrounding community.
- Maintain an in-depth knowledge of public health and health care, and use this knowledge to ensure successful program implementation.
- Work with multiple stakeholders, including patients, providers, government entities, and the community, to ensure successful program design and implementation.
- Provide guidance, support, and direction to program participants.
- Measure and evaluate program outcomes to ensure that objectives are being met.
- Create and maintain accurate records of program activities.
- Develop and maintain strong relationships with program participants, stakeholders, and other members of the community.
- Collaborate with other members of the health care team to ensure that programs are meeting their goals.
- Develop and present reports on program progress and outcomes.
- Participate in community outreach initiatives to increase public awareness about available health initiatives.
