Community Health Advocate

MedStar Health is looking for an experienced Community Health Advocate to join our team and advance our mission to provide quality healthcare to our community. The Community Health Advocate's primary role is to advocate for our patients by promoting health education, providing access to resources, and engaging with our community at large. Responsibilities:

• Advocate for patients through health education, resource access, and community engagement • Develop and implement health education programs for the community • Provide information and assistance to individuals in need of health resources • Work with community partners to build relationships and collaborate on health initiatives • Develop and maintain community health networks • Develop and assess health education materials • Monitor and evaluate program effectiveness • Identify community health needs and develop strategies to address them • Develop and present health education materials to a variety of audiences • Participate in outreach and community events • Document program activities and outcomes • Maintain up-to-date knowledge of health education and public health trends • Bilingual skills in English and Spanish preferred
