Job Description

MedStar Health is looking for an organized, empathetic, and detail-oriented Clinical Case Manager Discharge Planner to join our team. In this role, you will be responsible for developing and implementing discharge plans for patients, working closely with other members of the healthcare team to ensure smooth transitions to post-acute care settings. To be successful in this role, you must have a solid understanding of clinical case management, effective communication skills, and the ability to think critically and make timely decisions. You must also be familiar with healthcare regulations and standards, and have a genuine passion for providing the highest quality of care to our patients. Responsibilities:

  1. Develop and implement comprehensive discharge plans for patients.
  2. Collaborate with healthcare team members to coordinate smooth transitions to post-acute care settings.
  3. Monitor patient progress and ensure that all healthcare regulations and standards are met.
  4. Stay up-to-date on changes in healthcare regulations and standards.
  5. Provide the highest quality of care to patients.
  6. Review patient charts and medical records to create individualized discharge plans.
  7. Maintain accurate records and document patient progress.
  8. Respond to inquiries from family members and healthcare team members.
  9. Communicate effectively with patients and families about discharge plans and post-acute care options.
  10. Utilize problem-solving skills to make informed decisions in a timely manner.
