Job Description

Are you an experienced Registered Nurse with a passion for patient care and discharge planning? MedStar Health is searching for a skilled Case Manager (RN) Discharge Planner to join our compassionate and dedicated team. In this role, you will be responsible for ensuring that all patients at our facility receive the highest quality of care during their hospital stay and discharge.The ideal candidate for this role will have a Bachelor of Science in Nursing (BSN) degree and at least 3 years of recent clinical experience in a hospital setting. You must possess strong organizational skills and the ability to multitask, as well as excellent interpersonal communication skills. Additionally, you must be tech-savvy and able to use electronic health records (EHR) systems to coordinate patient care. If you have experience developing and/or implementing discharge plans, we highly encourage you to submit your application.

Responsibilities:

  1. Coordinate and provide patient care in accordance with established standards of practice and hospital policies.
  2. Develop and implement discharge plans for each patient.
  3. Monitor and document patient progress, including vital signs, treatments, and medications.
  4. Communicate with patients, families, and healthcare providers in order to coordinate care.
  5. Ensure compliance with all regulatory requirements and standards.
  6. Utilize electronic health records (EHR) systems to document patient care and discharge planning.
  7. Educate patients and families about discharge plans and post-discharge care.
  8. Participate in interdisciplinary rounds to discuss patient care plans.
  9. Work collaboratively with other healthcare professionals to ensure the highest quality of care is provided.
  10. Remain current with best practices and trends in healthcare, discharge planning, and related areas.

Job Qualifications
