Responsibilities:

1. Provide compassionate, person-centered care management services to patients, their families, and their support systems.
2. Develop and implement individualized care plans in collaboration with healthcare providers, patients, and families.
3. Monitor, assess, and evaluate patient progress.
4. Identify patient needs and develop strategies for addressing those needs.
5. Provide crisis intervention, emotional support, and resources to patients and families.
6. Facilitate communication between patients, families, and healthcare providers.
7. Maintain accurate and up-to-date records of patient care.
8. Participate in team meetings and provide feedback to multidisciplinary teams.
9. Stay current on best practices and standards of care.
10. Adhere to all applicable regulations and policies.
