Your role will be crucial in supporting individuals with mental health conditions by providing resources, advocating for their needs, and promoting wellness and recovery.We are looking for candidates with a Bachelor's degree in a related field and at least 2 years of experience in behavioral health case management. You must also possess a thorough understanding of mental health disorders, treatment modalities, and community resources.   1. Develop and implement personalized care plans for individuals with mental health conditions.
  2. Collaborate with patients, families, and interdisciplinary teams to address their mental health needs and promote wellness and recovery.
  3. Conduct assessments to identify individuals' strengths, needs, and resources.
  4. Provide education and support to individuals and their families on mental health disorders, treatment options, and community resources.
  5. Advocate for individuals' needs and rights within the healthcare system.
  6. Monitor and track individuals' progress and adjust care plans as needed.
  7. Ensure timely and appropriate referrals to other healthcare providers and community resources.
  8. Maintain accurate and up-to-date records of individuals' care plans and progress.
  9. Communicate effectively and empathetically with individuals and their families to establish trust and build positive relationships.
  10. Collaborate with insurance providers and other agencies to ensure individuals have access to necessary services and resources.
  11. Stay current with best practices and evidence-based treatments in the field of behavioral health.
  12. Adhere to ethical and professional standards in all interactions with individuals, families, and other professionals.
  13. Participate in team meetings and case conferences to discuss individuals' progress and address any challenges.
  14. Collaborate with community organizations and agencies to develop and promote programs and resources for individuals with mental health conditions.
  15. Maintain confidentiality and respect individuals' privacy rights at all times.

Job Qualifications
  • Bachelor's Or Master's Degree In Social Work, Psychology, Counseling, Or A Related Field.

  • Minimum Of 2 Years Of Experience Working In A Behavioral Health Setting, Preferably As A Case Manager.

  • Knowledge And Understanding Of Mental Health And Substance Abuse Disorders, Treatment Options, And Community Resources.

  • Strong Communication And Interpersonal Skills To Effectively Collaborate With Clients, Families, And Healthcare Professionals.

  • Ability To Assess And Develop Individualized Treatment Plans, Monitor Progress, And Provide Supportive Interventions To Promote Overall Well-Being.
