- Coordinate and manage the care of patients with complex behavioral health needs
- Advocate for patients and ensure their needs are met in a timely and effective manner
- Develop and implement individualized care plans for patients based on their specific needs and goals
- Collaborate with interdisciplinary teams to provide comprehensive care for patients
- Conduct thorough assessments to identify patients' needs and determine appropriate treatment plans
- Monitor patients' progress and adjust care plans as needed
- Connect patients with community resources and support services to enhance their well-being
- Serve as a liaison between patients, healthcare providers, and insurance companies to ensure seamless care coordination
- Educate patients and their families on mental health and behavioral wellness, and provide resources for self-care and self-management
- Maintain accurate and up-to-date records of patient interactions and treatment plans
- Stay current on industry trends and best practices in behavioral health case management.
Bachelor's Or Master's Degree In Social Work, Psychology, Counseling, Or A Related Field.
Minimum Of 2 Years Of Experience In Behavioral Health Case Management Or Related Field.
Knowledge Of Local And State Resources For Mental Health And Substance Abuse Treatment.
Strong Communication And Interpersonal Skills, With The Ability To Build Rapport With Clients And Collaborate With A Multidisciplinary Team.
Ability To Assess And Develop Treatment Plans, Monitor Progress, And Make Appropriate Referrals For Clients With Behavioral Health Needs.
