  • Coordinate and manage the care of patients with complex behavioral health needs
  • Advocate for patients and ensure their needs are met in a timely and effective manner
  • Develop and implement individualized care plans for patients based on their specific needs and goals
  • Collaborate with interdisciplinary teams to provide comprehensive care for patients
  • Conduct thorough assessments to identify patients' needs and determine appropriate treatment plans
  • Monitor patients' progress and adjust care plans as needed
  • Connect patients with community resources and support services to enhance their well-being
  • Serve as a liaison between patients, healthcare providers, and insurance companies to ensure seamless care coordination
  • Educate patients and their families on mental health and behavioral wellness, and provide resources for self-care and self-management
  • Maintain accurate and up-to-date records of patient interactions and treatment plans
  • Stay current on industry trends and best practices in behavioral health case management.

Job Qualifications
  • Bachelor's Or Master's Degree In Social Work, Psychology, Counseling, Or A Related Field.

  • Minimum Of 2 Years Of Experience In Behavioral Health Case Management Or Related Field.

  • Knowledge Of Local And State Resources For Mental Health And Substance Abuse Treatment.

  • Strong Communication And Interpersonal Skills, With The Ability To Build Rapport With Clients And Collaborate With A Multidisciplinary Team.

  • Ability To Assess And Develop Treatment Plans, Monitor Progress, And Make Appropriate Referrals For Clients With Behavioral Health Needs.
