1. Conduct comprehensive assessments of patients' psychosocial needs and develop individualized care plans in collaboration with the interdisciplinary team.
2. Provide counseling and support services to patients and their families, utilizing evidence-based interventions and techniques to address emotional, behavioral, and social issues.
3. Serve as a liaison between patients, their families, and other healthcare professionals to promote continuity of care and ensure that patients' needs are being met.
4. Advocate for patients' rights and collaborate with community resources to ensure access to necessary services and support systems.
5. Maintain accurate and timely documentation of patient interactions, progress, and treatment plans in accordance with departmental and professional standards.
6. Participate in multidisciplinary team meetings and provide input on patients' psychosocial needs and treatment plans.

8. Provide crisis intervention and support to patients and families during times of emotional distress or trauma.
9. Collaborate with other social workers and mental health professionals to develop and implement group therapy and support programs for patients and families.
10. Participate in quality improvement initiatives and assist in developing and implementing evidence-based interventions to improve patient outcomes.
11. Serve as a resource and consultant to other healthcare professionals on social work-related issues and provide education on social and psychological factors affecting patient care.
