If so, Mayo Clinic has an exciting opportunity for you as a Social Worker - LMSW for our Kidney Transplant team. As a social worker at Mayo Clinic, you will have the opportunity to use your skills and expertise to provide compassionate and comprehensive care to patients undergoing kidney transplant surgery. We are seeking a highly motivated and compassionate individual with a Master's degree in Social Work and LMSW licensure to join our team. Join us in our mission to provide the highest quality of care to our patients and their families.

  1. Conduct thorough psychosocial assessments of patients undergoing kidney transplant surgery to identify their individual needs and concerns.
  2. Collaborate with interdisciplinary team members to develop and implement personalized care plans for each patient, taking into consideration their physical, emotional, and social needs.
  3. Provide counseling and support to patients and their families throughout the kidney transplant process, including pre-transplant evaluations, post-operative care, and long-term follow-up.
  4. Assist in coordinating resources and services for patients, such as transportation, housing, financial assistance, and community support programs.
  5. Serve as a liaison between patients, their families, and healthcare providers to ensure effective communication and continuity of care.
  6. Advocate for patients' rights and assist them in navigating the healthcare system to access necessary services and support.
  7. Stay up-to-date on current healthcare policies and regulations related to kidney transplantation and advocate for necessary changes to improve patient care.
  8. Maintain accurate and thorough documentation of patient interactions and progress in accordance with departmental and regulatory standards.
  9. Participate in team meetings, case conferences, and educational programs to enhance knowledge and skills related to kidney transplantation and social work practice.
  10. Continuously seek opportunities for professional development and actively participate in quality improvement initiatives to enhance the overall patient experience.

Job Qualifications
  • Master's Degree In Social Work (Msw) From An Accredited Program

  • Licensed Master Social Worker (Lmsw) In The State Of Employment

  • Minimum Of 2 Years Of Experience Working In A Healthcare Setting, Preferably With Transplant Patients

  • Knowledge Of Federal And State Regulations Related To Transplantation And Healthcare

  • Strong Communication And Interpersonal Skills, With The Ability To Work Collaboratively With A Multidisciplinary Team And Provide Support To Patients And Families Throughout The Transplant Process.
