  1. Coordinate and manage the care of patients throughout their treatment journey, ensuring high quality and efficient care.
  2. Utilize strong clinical knowledge to provide expert guidance and support to patients and their families.
  3. Advocate for patients and ensure their needs are met by collaborating with interdisciplinary teams and other healthcare professionals.
  4. Develop and implement individualized care plans for patients, considering their unique needs and circumstances.
  5. Monitor and track patient progress and outcomes, making adjustments to care plans as needed.
  6. Act as a liaison between patients, families, and healthcare providers to facilitate communication and ensure continuity of care.
  7. Educate patients and families on treatment options, medications, and self-care techniques.
  8. Maintain accurate and up-to-date patient records and documentation.
  9. Stay current with industry best practices and regulations to provide the highest level of care to patients.
  10. Collaborate with insurance providers to ensure appropriate coverage and reimbursement for patient care.
  11. Participate in quality improvement initiatives to enhance patient outcomes and satisfaction.
  12. Serve as a mentor and resource for other healthcare team members, promoting a positive and collaborative work environment.

Job Qualifications
  • Bachelor's Degree In Nursing (Bsn) From An Accredited Institution.

  • Active Registered Nurse (Rn) License In The State Of Employment.

  • Minimum Of 2 Years Of Clinical Experience As A Registered Nurse.

  • Certification In Case Management Preferred (Ccm, Acm, Or Rn-Bc).

  • Strong Communication, Organizational, And Critical Thinking Skills.
