
Inpatient Internal Medicine Pharmacist

- Collaborate with physicians, nurses, and other healthcare professionals to ensure the safe and effective use of medications for inpatient internal medicine patients.
- Provide pharmaceutical care for inpatient internal medicine patients, including reviewing medication orders, identifying drug interactions and allergies, and recommending appropriate dosages.
- Monitor patient response to medications and make necessary adjustments to therapy as needed.
- Conduct medication reconciliation for new admissions and transfers, and provide education to patients about their medications.
- Keep accurate and up-to-date patient medication records, including documentation of interventions and outcomes.
- Participate in multidisciplinary rounds and provide input on medication-related issues.
- Stay current on new developments and advancements in internal medicine pharmacotherapy.
- Ensure compliance with all legal and regulatory requirements related to medication use.
- Act as a resource for other healthcare professionals regarding medication therapy.
- Provide guidance and mentoring to pharmacy students and residents.
- Collaborate with the pharmacy team to maintain adequate medication inventory and ensure timely delivery of medications.
- Participate in quality improvement initiatives and assist with the development of medication-related policies and protocols.
- Adhere to departmental and institutional policies and procedures.
- Maintain confidentiality of patient information in accordance with HIPAA regulations.
- Demonstrate excellent communication and interpersonal skills in all interactions with patients, colleagues, and other healthcare professionals.
Doctor Of Pharmacy (Pharmd) Degree From An Accredited Institution: This Is The Minimum Educational Requirement For An Inpatient Internal Medicine Pharmacist At Mayo Clinic.
