  1. Conduct comprehensive assessments of patients' psychosocial needs and develop individualized care plans.
  2. Collaborate with interdisciplinary teams to provide coordinated and integrated care for patients.
  3. Provide counseling and support services to patients and families to address emotional, social, and financial needs.
  4. Educate patients and families on available community resources and assist with accessing these resources.
  5. Advocate for patients' rights and assist with navigating complex healthcare systems.
  6. Collaborate with healthcare providers to ensure patients' treatment plans align with their social and emotional needs.
  7. Maintain accurate and up-to-date documentation of patient interactions, progress, and treatment plans.
  8. Participate in patient and family care conferences and provide recommendations for ongoing care.
  9. Stay informed about current social work practices, policies, and regulations to ensure compliance and best practices.
  10. Provide crisis intervention and support for patients and families in emergency situations.
  11. Develop and implement discharge plans to ensure patients have appropriate support and resources upon leaving the hospital.
  12. Serve as a liaison between patients, families, healthcare providers, and community agencies.
  13. Collaborate with insurance companies and other third-party payers to secure necessary coverage for patients' care.
  14. Participate in quality improvement initiatives and provide input for enhancing social work services.
  15. Maintain strict confidentiality and adhere to ethical standards in all patient interactions.

Job Qualifications
  • Master's Degree In Social Work From An Accredited Program

  • Minimum Of 2 Years Experience In A Healthcare Setting, Preferably In An Inpatient/Acute Care Setting

  • Current Licensure As A Licensed Clinical Social Worker (Lcsw) In The State Of Employment

  • Knowledge Of And Experience With Mental Health And Substance Abuse Treatment, Crisis Intervention, And Discharge Planning

  • Strong Communication And Collaboration Skills, With The Ability To Work Effectively With Interdisciplinary Teams And Diverse Patient Populations.
