The ideal candidate will have a Master's degree in Social Work, Counseling, Psychology, or a related field, as well as a minimum of two years experience in the mental health field. Responsibilities:

1. Act as a bridge between the hospital, outpatient providers, and community resources to ensure patients receive the best care possible.
2. Develop and implement strategies to improve access to mental health services.
3. Establish and maintain relationships with mental health providers in the community.
4. Coordinate patient care across multiple departments and providers.
5. Monitor and assess patient progress and outcomes.
6. Provide education and support for patients and their families.
7. Develop and implement programs and services to meet the needs of patients.
8. Evaluate and modify treatment plans as needed.
9. Provide crisis intervention services when needed.
10. Participate in quality improvement and research activities.
