- Develop and implement evidence-based medication therapy plans for patients.
- Conduct drug utilization reviews to ensure safe and effective use of medications.
- Provide education to patients and healthcare professionals on medication therapy and proper use of medications.
- Utilize clinical knowledge and expertise to support whole-person care for patients.
- Collaborate with healthcare team members to ensure proper medication management and patient safety.
- Serve as a resource for pharmacotherapy and drug information for healthcare professionals.
- Maintain accurate and complete patient medication records.
- Adhere to all state and federal regulations related to pharmacy practice.

- Stay updated on new medications, treatment guidelines and advancements in pharmacy practice.

Doctor Of Pharmacy (Pharmd) Degree From An Accredited School Of Pharmacy.
Current And Active Pharmacist License In The State Of California.
Completion Of A Post-Graduate Residency Program In Clinical Pharmacy.
Board Certification In A Relevant Area Of Specialty (E.g. Ambulatory Care, Critical Care, Oncology).
Minimum Of 2 Years Of Experience As A Clinical Pharmacist In A Hospital Or Healthcare Setting.
